Sheboygan County, Wisconsin Aging and Disability Resource Center

Enabling Resolution:

WHEREAS, by the adoption of Sheboygan County Resolution No. 30 (2004/05), the Sheboygan County Board authorized the Sheboygan County Health & Human Services Department to apply for a contract with the State of Wisconsin to operate an Aging and Disability Resource Center (ADRC), and

WHEREAS, the State of Wisconsin has agreed to contract with Sheboygan County for this purpose and has notified the Health & Human Services Department that it is awarding the Health & Human Services Department the sum of $332,415 to fund the ADRC operations this year with similar or enhanced funding anticipated thereafter, and

WHEREAS, the State funding is to be used primarily for staffing and purchase of equipment, promotional materials, and operational expenses; and it will be necessary to add three positions to the Table of Organization (T/O) to operate the ADRC which will be funded out of the $332,415, and WHEREAS, it is proposed that the three positions to be created would be an Aging and Disability Resource Center (ADRC) Supervisor, a Social Worker I, II, III, III MA/MS, and a Disability Benefit Specialist, all positions which will sunset at such time as state funding is no longer available, and

WHEREAS, the Health & Human Services Department proposes to reassign some of its current staff which currently serves the aging and disability population to operate within the broader structure of the ADRC, all under the supervision of the Manager of the Division of Community Programs, and WHEREAS, in order to fully implement the ADRC concept and to provide the most efficiency for the aging and disability service population, the Health & Human Services Board proposes that the T/O be changed as set forth below;

NOW, THEREFORE, the County Board of Supervisors of the County of Sheboygan does ordain as follows:
